Hotel Reservations Update (September 16, 2006):   Our conference hotel is completely booked for several nights during the AGM, including Saturday.  No need to worry though.  We have set up overflow room blocks at two hotels near Loews with special rates available until September 26, subject to room availability. Visit the Overflow Hotel Information page to learn about the hotels and how to make reservations.

Please Note: If you reserved more room nights at Loews than you will actually need for the AGM, or if you have decided to room with someone and still have an extra room reserved, please cancel the room nights you won’t be using.

New for the 2006 AGM

  • "Dine-around" shuttles will be chartered for Thursday and Friday evening. Tickets must be pre-ordered on the AGM registration form. Learn more and view a list of the restaurants on the shuttle routes.

  • An optional sandwich buffet will be available Friday and Saturday as a quick alternative to eating lunch in a restaurant. Tickets must be pre-ordered on the AGM registration form. We HIGHLY recommend this option, since the hotel restaurants will not be able to accommodate all AGM attendees for lunch at the same time, and the nearest off-property restaurants are about a mile from the hotel. View the buffet menus.

The Registration Forms

Although AGM and Tour registration forms were included in the brochure mailed to all JASNA members, we would prefer that you fill out the forms provided in Adobe Acrobat PDF format below. (Click on the turquoise "AGM Registration Form" and "Tour Registration Form" boxes below.)

Using these forms, you will be able to easily type in your information, and the form will automatically calculate your total fee. After completing a form, please print two copies, keep one for your records, and mail in the other with your check or money order. Members who register for the AGM using the forms on the website will be entered in a drawing for a special raffle basket. These forms must be mailed; they cannot be submitted online.  Also, the Tour Registration Form must be mailed separately to the tour provider, Convention & Group Services.

The registration forms can only be opened if you have Adobe Acrobat Reader software on your computer. You can download a free copy of the most recent version of the software by clicking on the Adobe Acrobat Reader link above. Additional tips for using the forms on this website are provided below.

Tips for Filling Out These Forms

  • To enter text on a blank line, just move your cursor down the page towards the line. You don't have to align your cursor, because the text entry points are pre-set, as are the font styles and sizes. As you approach a blank line you will see the cursor turn from a hand to an I-beam pointer.  When this happens, click your mouse. A blinking insertion point will appear in a pre-designated spot, and you can simply begin typing. When done finished, just drop down to the next line with your mouse or press the Tab key.

  • The dollar amounts you enter will total at the bottom of the form. However, you will need to enter the correct amount on each line and multiply the amounts yourself if you purchasing tickets, etc. for more than one person.

  • Text can be removed by hitting the Escape key. Check marks can be removed by clicking on them or pressing the Ctrl and Z keys at the same time.

  • When you are ready to print out your completed form, print from the Adobe program (e.g., use the Print icon in the Adobe program). Don't use your browser's Print icon.

  • If you have problems getting the I-beam cursor to appear or if you have problems printing, you might want to upgrade to the latest Adobe Reader program using the link above.

  • Special note for Mac OS X users:  If the registration forms open, but you can't fill them out, look in the upper left corner to see whether it says "Adobe Reader" or "Preview".  If it says the latter, your computer has opened the form in an image viewer program because it couldn't find Adobe Reader.  If you can't find the Adobe Reader program either, download it free from the Adobe website.  If you do find Adobe Reader on your computer, open the program, go to the File menu, click on Open, then navigate to the form (registration_form.pdf or tour_form.pdf) and open it from within Adobe Reader. (The form has presumably been already downloaded onto your computer.) You should now be able to fill it out.
